What I tend to use is “ terrain.party”, “Google Map Customiser”, Fireshot, and Photoshop / Gimp. Its pretty simple to make a custom map in Cities : Skylines, but a couple of the steps are a bit fiddly (namely using the Google Map Customiser its not so intuitive).
